Family Survey - Early Intervention
Please help the Massachusetts Department of Public Health (DPH) to improve the services you and your child are getting from a Massachusetts Early Intervention program. Please take a few minutes to complete this survey about how EI has helped you and your child. Your responses are confidential and will be used to help DPH understand if EI is making a positive difference for your family. Completing this survey also helps your EI program improve the services families receive.
